“Why Did You Cut Off…” Pilot’s last chat, AAIB’s preliminary report reveals detail | Air India Crash

July 12, 2025 (ANI): First report on the horrific Ahmedabad crash of Air India’s Boeing 787 released by Aircraft Accident Investigation Bureau. AAIB’s preliminary report disclosed the crucial dialogues exchanged by the pilots just before the crash. Report outlines a harrowing sequence of events that unfolded within 90 seconds of the take-off. Both engines shut down unexpectedly during initial climb, leading to catastrophic loss of thrust and rapid descent, report said. Pilots’ conversation was recorded in the aircraft's Enhanced Airborne Flight Recorder during the crash. “Why did you cut off?”, one pilot was heard asking the other, to which the response was, "I did not". Recovered flight data suggested, fuel cut-off switches for both engines surprisingly moved from RUN to CUTOFF within one second. Uncommanded shutdown triggered deployment of Ram Air Turbine (RAT); thus aircraft began losing altitude almost immediately. Final distress call – "MAYDAY" – transmitted at 08:09 UTC, just seconds before the aircraft crashed. The crash killed 260 people, including 229 passengers, 12 crew members, and 19 people on the ground. Report suggested that no technical defects linked to the engines or flight control systems prior to accident. Both the pilots were fully qualified and had no recent duty irregularities or medical issues reported. FAA, in 2018, issued Special Airworthiness Information Bulletin warning over fuel control switch on similar Boeing aircraft. Air India, however, did not perform the recommended inspections in the advisory, said AAIB’s report. Air India posted on ‘X’ as it “acknowledged receipt of preliminary report” by AAIB. Given the active nature of the investigation, we are unable to comment on specific details, Air India further said. Notably, AAIB has not issued any immediate safety recommendations, as further assessments are underway.
